Signal Failure

A malfunction in the signaling system that prevents signals from displaying correct aspects, typically causing trains to stop or proceed at severely reduced speed under manual authority. Signal failures are one of the most common causes of railway delays. Fail-safe design means signals default to their most restrictive aspect (red/stop) when a fault occurs.
